Registration By MailPlease complete registration form (see below). Do not omit the "Day, Class, or Time." Mail application with first and last months' payments (made payable to Wendy Worth), and a $50.00 ($20 for Summer classes) registration fee to: Wendy Worth No student will be registered until their registration form has been signed (registration forms MUST be signed or student will NOT be admitted to class), and a $50.00 registration fee has been paid. Minimum enrollment of six students is required for all classes. CostsPayments are due the first class of each month. We feel that it is important to finish what is started and to help ensure this policy, we ask that you take the first month on a trial basis. After the first month you are committing to complete the season.
Students will be charged for the full amount of tuition whether or not they remain to the end of the year. There is a yearly registration fee of $50.00 per family. There will be NO REFUNDS. Missed classes may be made up in any alternate class approved within 3 weeks after absence or forfeited. Cancellation of private lessons must be made within 24 hours prior to scheduled lesson or full payment is required. Late Fees:Late payments over 1 week will be charged a $5.00 fee per week. Returned Checks:$25.00 charge plus bank fees.
